On Wednesday, October 11, 2023, at 6:05 p.m., the Oregon State Police responded to a single vehicle crash on Hwy-138E, near milepost 21, in Douglas County. The preliminary investigation indicated a Ford F-350, operated by Douglas Floyd (70) of Middleton (ID), was traveling westbound when it lost control, left the roadway and struck a tree. The occupants of the vehicle were extricated by first responders and transported to a local hospital. The operator (Douglas Floyd) was declared deceased after being transported to the hospital. A passenger in the Ford, Kimberly Colleen Floyd (62) of Middleton (ID), suffered minor injuries. The highway was not impacted during the on-scene investigation. OSP was assisted by Glide Fire, Umpqua Ambulance, and ODOT.
